Direct Deposit Change Form

Changes to your direct deposit can be made through your Skyward Employee Access account or you can print off the Authorization for Direct Deposit form below and bring it to the Payroll office. All changes made after the 10th of the month will take affect the following month. Please do not close your bank account until you confirm the changes are complete.
